What do you need to have ready when you apply?

Your checklist (if working is involved):

❏ Your personal details
❏ Details about your disability/health condition and how
it affects you
❏ Your workplace address and postcode
❏ The name of a workplace contact who can authorise your Access
to Work payments, their email address and work phone number
❏ (Only if you’re self-employed) Your unique tax reference number

If your employer does not yet know about your intention to use Access to Work for support, that’s absolutely fine! It’s not mandatory to enter this information.

It’s a good idea to write down in detail how your disability affects you at work, and what you think might help. Access to Work will phone you, often out of the blue, so it’s best to have this information on hand.[/vc_column_text][/vc_tta_section][vc_tta_section title=”What is the application timeline?” tab_id=”1619947072321-184a64ce-770b”][vc_column_text]

Timeline for Access to Work

Step 1 – Apply for Access to Work, using the online application form or by phone

Step 2 – An assessor will be assigned to you within 3-4 weeks of submitting your application. They will call you to discuss your application and to determine next steps.

Step 3 – The assessment (varies based on individual needs): You will be required to find 3 quotes from suppliers, including Connections in Mind, for the services you need. We can help with these.

Step 4 – The assessor feeds back their findings to DWP, including medical reports/assessments. DWP decide on the amount and duration of funding.

Step 5 – You will receive an offer letter from Access to Work stating the nature and amount of the funding. There is a declaration letter that you must sign and return.

Step 6 – You will be allocated a sum of money to fund coaching and we can help you find the best service to suit your needs within this budget.

Step 7 – As you come to the end of your grant, you can always apply for a renewal, or additional funding.[/vc_column_text][vc_empty_space height=”20px”][vc_btn title=”Download ATW Timeline” link=”url:https%3A%2F%2Fdrive.google.com%2Ffile%2Fd%2F1FbSR2jJikYnb66FT9Rko4Tnbc6U51GGl%2Fview”][/vc_tta_section][vc_tta_section title=”Your application crib sheet” tab_id=”1619945262981-c40fd8e2-24f9″][vc_btn title=”Download the ATW Crib Sheet” link=”url:https%3A%2F%2Fdrive.google.com%2Ffile%2Fd%2F1tmvFKqyGOCkUYW3y9B5e8WWS9edn-Ema%2Fview”][/vc_tta_section][vc_tta_section title=”Organising your payments” tab_id=”1637155655842-b161be6d-1f83″][vc_column_text]

Organising your payments

There are two ways to receive Access to Work funding. When you receive the grant confirmation letter from Access to Work, you will receive a ‘DP228 payment info form’, where you must fill out your personal details as well as the payee details.

Option 1
Give Connections in Mind’s payment details so that Access to Work pay us directly. If you choose this option, you will have to send a copy of our invoice and a ‘DP222 claim form’ to the address stated on the form each month.

Option 2
Provide your own payment details for Access to Work to pay you directly.
If you choose this option, you would pay our invoice each month, then send your receipts to DWP along with the claim form, and they would reimburse you. The address to send these forms to will be on the back of your form. If you are self employed, Connections in Mind will need to countersign your forms to agree that coaching was provided.[/vc_column_text][/vc_tta_section][/vc_tta_tabs][/vc_column][/vc_row][vc_row][vc_column width=”1/3″][vc_cta h2=”Email us”]info@connectionsinmind.co.uk[/vc_cta][/vc_column][vc_column width=”1/3″][vc_cta h2=”Call us”]+44 (0)208 050 1605[/vc_cta][/vc_column][vc_column width=”1/3″][vc_cta h2=”Speak to us”]BOOK AN APPOINTMENT[/vc_cta][/vc_column][/vc_row]
